Do you also have a lot of overjet? (more commonly referred to as overbite) You might be a good candidate for orthognathic surgery. Your teeth get lined up with braces and then an oral surgeon moves your lower jaw forward to where it should be. The surgeon can also do other things such as platysmal plication or submental lipo to augment the look. You would need to be in braces to get ready for the surgery.
